Welcome to When It Goes Wrong, the podcast about disasters, accidents and when things fall apart. On this episode I discuss two airplane crashes - Air France 447 where pilots failed to manage with mechanical failures, and the Lake Constance mid-air collision where two planes crashed due to a conflict between air traffic control and plane systems. I discuss how the hand off between technology and humans is becoming increasingly important with the rise of AI.
